First, sketch your pattern and place plastic film over it.  I use page protectors for the big pieces.  You can use wax paper for smaller pieces, but be careful because wax paper wrinkles.
Then, outline with royal icing.  Thin icing with a few drops of water.  A good test is to draw a knife through a bowl of thinned icing.  The line should disappear after a count of 10.  Pipe thinned icing till it flows to piped lines.

When the white color flow has dried, I use red to fill in the ribbon, bow and hearts.

You will notice that the bow and pink hearts were piped separately.  This will add a 3-D effect when the card is assembled.

The teddy bears were painted with powdered food coloring and cooking oil using a fine tip brush.

When everything is dry, fasten hearts, bow and bears with a small amount of icing.  Dots were piped around the inside of the heart cut out in a pyramid shape.  With a #0 tip, pipe message on card.  Using a #4 tip, pipe 2 thick lines - one on top of the other along the edge and attache the front to the back.  Support the card with a can of food until it is dry.  For extra support, I piped a color flow disk around the standing card.  Take my word for it.... it's highly recommended to add this support.  I didn't the first time and the card fell over and shattered like glass, and I had to start all over again.
